Animal Registration Online Form

Before You Start

Some sections of this form ask you to upload a file.

 

You will need:

  •  Microchip Number Certificate copy is required for all new registrations (scan or photo)

You may need:

  • Pension Card copy for pension card holders (scan or photo)
  • Sterilisation Certificate (scan or photo) or
  • A copy of registration of an applicable organisation

  • A copy of certificate obedience trained with recognised organisation

  • Evidence that dog/cat kept for breeding Domestic Animal Business

  • Statutory Declaration (scan or photo) if your dog or cat:
